Superior Home Health Care will send a registered nurse to your home to assess your condition and determine if home health care is needed. Once assessment is completed, the registered nurse will notify your personal physician, and a plan of care will be established according to your needs. You will then be assigned a nurse who will see you as ordered by your physician. Your assigned nurse will maintain continuous contact with your physician to make sure that your plan of care is carried out and updated as needed.
